Centralization of Community Health Outreach Programs (CHOP) Health Fairs

 ATTENTION EVENT PLANNERS:

Please help Miami-Dade County become more effective in reaching all areas of our county and get more organizations to partner with you on your event!

1. Check our Event Calendar to make sure that the date is clear for your event.

2. Download and use our CHOP Health Fair Form to request that your event be posted on the calendar. The form will be shared with the Consortium for a Healthier Miami Dade’s distribution list of service providers. The Consortium members will then contact you directly to discuss their availability to participate in your event.

The purpose of this form is only to disseminate information about community health fairs and provide a forum for collaboration and partnerships between interested organizations. It is NOT the responsibility of the Consortium to coordinate or determine if there is any sponsorship exclusivity related to each community health fair. ATTENTION CHOP SERVICE PROVIDER:

Make your life easier!

· Use our CHOP Service Encounter Form to summarize your data. It will be compiled to summarize the results of the event. Your organization’s data will be available for your review.

· MEMBERSHIP BENEFIT: If you are a Consortium Member you will be able to obtain a complete summary of all service encounter statistics provided to the Consortium for each event you participate in. Become a member register NOW!

· COMING SOON! Coordination of Referrals from CHOP Health Fairs/Events.  The goal is to develop a uniform mechanism for referring Health Fair/Event Participants with undesirable or abnormal findings to receive follow-up care with a health care provider. The committee is currently working with our local

providers to pilot a referral mechanism to make sure that all individuals with abnormal findings are referred for follow-up care and those without a medical home will be referred to community health centers and free clinics.
